Impact:In Audit Program page, while creating a new audit program it displays unwanted alert. Expecting this alert while Modifying the Audit Form not for creating a new Audit Form

Steps To Reproduce:
1)login to module as admin
2)create a audit type
3)click new to create audit program
4)in assign audit type and forms
5)click on + icon
6)enter form name
7)observe the screen
Description
In Audit Program page, while creating a new audit program it displays unwanted alert. Expecting this alert while Modifying the Audit Form not for creating a new Audit Form.
This Alert is recently added in cycle 4.
