closedDocument Template: Without having options for checkbox,select,multi select and radio button, the element should not be saved. Due to that i cant able to close the additional info popup in new document request

Steps To Reproduce:
1.Add checkbox to the doc template and save it w/o adding options
2.Click on new document request
3.attach the document and click on additional info plus icon
4.Enter the details, since the option not created for checkbox, it will show the element in additional info popup
5.ENter all other details and try to close the popup, it wont allow
