In Training Records page, setting up of questionnaire is not working as expected.

Steps To Reproduce:

1. Log in as Admin.
2. Go to consolidation menu.
3. Create new with 2 surveys.
4. Go to records page.
5. Select the created training.
6. Select questionnaire for any 1 survey.
7. Give date and percentage.
8. Click on save and observe

Description

In Training Records page if there is 2 types of surveys assigned for a training, then if the user selects questionnaire nd details for any 1 survey and clicking on save allows to save the record.
