Steps To Reproduce:
1. Login to EwQIMS.
2. Navigate to APQP module.
3. Create a project and add a task.
4. Assign the task to a resource.
5. Assign Adhoc approver for the task.
6. Attach an input document.
7. Login as task assignee and attach output document.
8. Login as approver and click on document for approval.
9. Provide second password and click on Submit button.
=> Error: Object reference not set to an instance of an object.
