Steps To Reproduce:
1.Login to PS
2.Click on actions→Initiate problem solving
3.Click on view form(8d)
4.right click add action
5.Close form
6.Open track project
7.Delete the action from 8d form
8.Go back to 8d form
9.Delete the deliverable
10.Observe result
