Steps To Reproduce:

1. Login as auditee
2. Click on corrective actions link from menu
3. Click on auditee path hyperlink
4. Select any car for a finding
5. Click on attachment field
6. Click on browse button
7. Select the document from local machine
8. Click on attach button
9. Observe the result
