Steps To Reproduce:

1. Login EwQIMS as Maintenance Person Rights
2. Click on Activities tab
3. Click on Breakdown Work order List and Work order Menu
4. Click Add button on Breakdown Activity
5. Observe the design
6. Enter the data
7. Click on Save button
8. Observe the Add to Activity Master field
