Steps To Reproduce:

1.Login
2.Click on problem solver
3.Actions→New concern
4.Click on attach files
5.click on attach document icon
6.select file click on done button
7.Now attachment has been saved
8.click on attach files again
9.Close the window
10.save concern
11.previously attached file not available
