Steps To Reproduce:

1. Login as auditor
2. Click on perform audit link from menu
3. Click on auditee hyperlink
4. Click on upload NC /OFI
5. Enter the data in all fields and save it
6. open the same NC or OFI by clicking on respective hyperlink
7. Observe the result
