Spare Part Delivery: Reason field is not there in Item Request Entry field

Steps To Reproduce:

1. Login EwQIMS as TPM Module Administrator
2. Click on Transaction tab
3. Click on Spare Part Delivery Menu
4. Click on Created Work order Code
5. Select the Row and click on Edit button
6. Enter the Reason
7. Click on Submit button
8. Observe the Result

Description

Issue 1: Spare Part Delivery: When clicking in to the Submit button, the system displays the "Error msg" - Closed

Issue 2: Entered Reason not getting saved.

(Impact)Reason field is not there and total cost detail getting updates in the field which is not having the header details.
