Steps To Reproduce:

1.Open the Browser
2.Enter the URL
3.Enter the Username and Password
4.Click on Login Button
5.Click on Module icon
6.Click on AQuAPro
7.Click on Main Menu
8.Select the PI -> Open PFMEA Document
9.Select the "Potential Effects of Failure: Sev" Column
10.Double-Click on the Cell which is Pasted Design Failure Value having ":" Symbol
11.Give backspace for the added ":" and Click outside the Cell immediately
12.Observe the Results

Description

In "Potential Effects of Failure: Sev" Column, while editing the added Design Failures option by giving Backspace in the Cell or Clicking outside the Cell its displaying as "Cannot insert duplicate key row in object 'dbo.AssocInstJoin' with unique index 'IX_AssocInstJoin2'. The duplicate key value is (28955, 28228, 113, 28029, <NULL>).
The Statement has been terminated"

Cannot replicate it. Probably this might have been got fixed by fix for #8224
