1. Login as auditor
2. Go to perform audit page
3. Click on any one audit hyperlink
4. Click on upload audit record
5. Observe the result form form type field

Description

Perform Audit -> Upload Audit Result -> Assigned Form type is not getting displayed for a form

Note: The user assigned Form type as "Template" but in upload audit result page its displaying as "Reference" with * mark
