Steps To Reproduce:
1. Login as auditor
2. Click on Perform audit link from menu
3. Click on any one audit cycle / auditee hyperlink
4. Click on add button under audit attachment tab
5. Select attachment type as "Controlled"
6. Attach a document
7. Observe the result
