Cant able to Create OFI in Perform Audit page.

Steps To Reproduce:

1. Log in as Auditor.
2. Go to perform Audit menu.
3. Pick any of not performed audit.
4. upload Audit Records.
5. Upload 2 NC and publish.
6. After approval create OFI.
7. Observe the result.

Description

Cant able to Create OFI in Perform Audit page. After filling mandatory fields and saving shows saved successfully but it is not listing in listing page.
