Steps To Reproduce:

1. Log in as Admin.
2. Go to Module setup,
3. Click on Audit Program Menu.
4. Create new Audit Program.
5. Go to Audit type and form tab.
6. Add Type and form.
7. Click on Add in RHS and observe the screen

Description

In Audit Program -> assign audit types and form page, While attaching document for Template form,shows two Done button but after clicking on first done only document is getting attached. Directly clicking on done button in pop up not getting attached.
