Add custom elements to the concern creation form and concern initial evualation form
create a new concern and enter data in the custom elements
Click on concern initial evualation page
data from concern creation page will not be transferred

Description
1) data values entered in the custom elements in concern creation form are not transferred to the concern initial evualation form. Same occurs to initate problem solving
2) When entering the new data ion multiline, date and textbox, data are not saved.
