1.Login into EwQims
2.Go to Problem solver module
3.Actions→Intial Evaluation Completed
4.Click on View Form
5.Click on 8D
6.Add 3 Rows of data , in action description
7.Delete 3rd row , delete 1st row
8.Check Screenshot
