Steps To Reproduce:
1. Log into audit pro as scheduler.
2. Go to Module setup.
3. Access the audit program menu and go to Audit types and form mapping page.
4. Select a type and right click on selected type.
5. Select form type as template.
6. Attach a file.
7. Click on done and observe
