Steps To Reproduce:
Refer description
1.Create a new concern and submit
2.check concern available in concern initial evaluation
3.now click on problem solver setup →define folder structure
4.set folder structure
5.go back to initial eval and submit the concern
6.Click on initiate prob solving click on concern
7.select forms click on publish
8.Observe result

Note: Docpro level and folder structure is defined
*Issue observed in all browsers
Observation : Faced this issue after setting doc-pro folder structure and tried to publish
CYcle 3 : Issue occur in SI13 Concern*
