Problem on selecting name of the supplier from "Vendor Facility audit form"

Steps To Reproduce:

1)Login to module as admin
2)click on supplier management menu
3)click on vendor facility audit
4)click on open new form
5)enter according as mentioned in the description

Description

1)Displays number after selecting the Inactive supplier from suite
2)Location is not populating automatically
3)Product supplied to JIC is not allowing to put in any data
4)Date field After selecting the date displays JS error
