Bug #4649

After mapping the questionnaire, the user will go and save the Survey in the Evaluation page. Then the Questionnaire will be viewable in the Participants action page, But now it is not getting displayed in the action Page

Steps To Reproduce:

1. Log in as Admin.
2. Go to Records page.
3. Map questionnaire to Post test survey.
4. Go to Evaluation page.
5. Click on ">>" icon in by questions column.
6. Click on save.
7. Log in as any of the participants.
8. Go to Action page and observe
