Steps To Reproduce:

1.Login to Aquapro.
2.Select any PI ->select PFMEA Document ->Right click publish
3.webdialog appear ->Fill all the text field ->click ->OK
4.Then go to docpro and see the respective folder.
5.Right click ->see revision history.
6.look into the Reason for change grid.
7.Observe result.
