Steps To Reproduce:
1)login to module as auditor
2)click on perform audit
3)click on re upload extension
4)raise a request for first time
5)click on submit after selecting all the mandatory fields
6click on "Re upload extension request history" and observe
7again click on "re upload extension" raise the request for the same date again.
8observe the screen after request is raise after second time by clicking on "Re upload extension request history"
