closedImpact: In Audit form mapping page, if a user adds a form but there is no attachment or Link is provided refreshing the page not removes the audit form from the List but form type changed to non editable one

Steps To Reproduce:
1. Log in as Administrator.
2. Go to Module setup.
3. Click on Audit program menu.
4. Click on Audit form tab and go to audit form mapping.
5. Add new form and select audit type as template and give link instead of attachment.
6. Also try the same in Online form type.
7. Refresh the page the observe the result.
