Steps To Reproduce:

1. Login EwQIMS as Module administrator
2. Click on the PDF template
3. Click on add and uncheck all the checkbox except Revision history summary
4. Observe the extra field on top of the Revision history details and select the Page size and layout and save
5. Obesrve the created template

Description

Issue 1: Schneider->PDF Template-> Unwanted field is showing if we unchecking all the check box except Revision History summary

Issue 2: User selected page size (A3) and Layout (Landscape) is not getting saved after saving the template and view the details
