Steps To Reproduce:

1.Click on New Document Request
2.Select a Level (where Include Template is Disabled)
3.Upload Documents
4.Select a Template from Template Selection dropdown
5.Click on Add
6.Click on Documents menu
7.Select a Level
8.Right click on Document Number Link - Print PDF
9.Template is not appended on Document

Description

If Include Template Disabled - Then on selecting any Template in New Doc Req, it's Not Appending to Document (Note: Facing this Only on JPG, HTML, PPT, TXT, XL formats)
