Steps To Reproduce:
1.Login to EWQIMS
2.Click on change management left menu
3.Navigate to initiate change request submenu and click on it
4.Select category from the dropdown
5.Click on initiate change request button
6.Navigate to change request status
7. Open the initiated Memo form
8.Check for the timeline on the top of the form
