openSystem is allowing to Review the Document from TOC-Document Review option while the Document is sent for Document Reviewers

Issue01:
1.Login as Document Owner
2.Actions - Document Due for Review
3.Select Document Reviewer and Continue
4.Navigate to Document TOC - Right click on Document Number Link - Select Document Review
5.Select No Change Required and Continue
FAIL - Action is getting Removed from Assigned user's "Document Review" Action
Issue02:
1.Navigate to Document TOC - Right click on Document Number Link - Select Document Review
2.Select Request for New Revision - Fill all Mandatory fields and continue
3.Actions - Document Due for Review - Select Document Reviewer and Save
4.Login as Document Reviewer - Actions - Document Review
5.Here 2 Actions are showing for same document of Rev2
Description
Issue01:
In Document Due for Review User Action - On selecting Document Reviewers it will be removed from Action and sent to Document Reviewers Review. So that Document Owner can do next Review only after Document Reviewers Complete their Review.
But now due to TOC-Document Review Option, once the Action is waiting in Document Reviewer Action List, Owner can able to Request for new revision or give as No Change Required
Example:
1.For a New Document, From Document Due for Review User Action - Select Document Reviewer and Save
2.When Document Reviewer has Not Reviewed it yet, As Document Owner go to Documents TOC and click on No Change Required
3.Now, that Action is getting Removed from Assigned user's "Document Review" Action
Issue02:
After the Above Steps in Example,
1.TOC-Document Review - Do Request for New Revision
2.Document Due for Review User Actions - Select Document Reviewer and Save
3.Now login as the Document Reviewer User. Here in Document Review Action menu, 2 Actions are Showing for same Rev2(Duplicate)

This is not at handled. Moving to New feature project
