openDocument Review History - Only the Latest Log of that Revision is showing, need all the Logs of that Revision in Separate Rows

Issue01:
1.Navigate to Documents TOC
2.Right click on Document Number Link - Select Document Review
3.Select No Change Required
4.For the same Document again Right click on Document Number Link - Select Request for New Revision - Fill all Mandatory fields and Continue.
5.Navigate to Document Management - Right click on a Level - Select "Document review history"

Issue01:
Document Review History - Only the Latest Log of that Revision is showing, need all the Logs of that Revision in Separate Rows.
For EG: For Doc of Rev1, I gave No Change Required from TOC-Document Review and then did Request for New Revision.
Currently its showing as,
1.Rev1 - Status "New Revision"
2.Rev2 - Status "Document Action Required"
Expectation:
1.Rev1 - Status "No Action"
2.Rev1 - Status "New Revision"
3.Rev2 - Status "Document Action Required"
Issue02:
For EG: For Doc of Rev1, we gave No Change Required from TOC-Document Review and then did Request for New Revision.
Log is shown as below.
1.Rev1 - Status "New Revision"
2.Rev2 - Status "Document Action Required"
Then, for Rev2 I gave No Change Required from TOC-Document Review, now the status of Log on Rev1 is changing from "New Revision" to "Document Action Required"
Log is shown as below.
1.Rev1 - Status "Document Action Required"
2.Rev2 - Status "No Action"
