Steps To Reproduce:
1. Login as AQuA Pro Module Administrator
2. Click on the Products Tab and select Design VDA Menu.
3. Select the Failure Analysis menu
4. Select a Production Item
5. Right-click on the Design Cause or Flowdown element
6. Select Risk
7. Add Preventiv/Detective and click on ok button
8. Observe the result

When I add risk and optimization after the name, the system displays different names like (OR=1 and DR=1).
