Steps To Reproduce:
In Multi Site:
1.Navigate to TOC - Click on a Level.
2.Right click on a Document - Select Add Record.
3.Attach a Document and Enter Record Description.
4.Click on Continue.
5.Navigate to TOC - Click on a Level - Observe the Document in which we added Record.
Document is showing Twice in TOC.

Description
In MULTISITE -- TOC - Document is getting Duplicated once we Add Record to the Document through Module Auto Approval or Route Approval.
Note: This Issue is Not Replicating in Corporate.
