Steps To Reproduce:

1. Login as PM
2. Open any project and add Risk
3. Risk action will display for the assigned person's action management page
4. Check the behavior, all pending risk actions date and time will be updated with current date and time and whenever refresh page, action created date will be updated with current date and time
