Steps To Reproduce:

1.Login as CM module admin
2.Select Change management menu
3.Click on initiate Change Request 
4.Select the PCR Form mapped category and Proceed
5.navigate to 2nd section Scope of Change in the Form (PCR)
6.check for the checkbox's
7.observe changes

PCR Form - Not able to check checkbox's in scope of change section, while selecting check box its shows " sql data exception "
