Steps To Reproduce:

1.Login as CM module admin
2.click on change management menu
3.navigate to horizontal deployment menu and click on it
4.select particular row and click on view horizontal deployment icon
5.click on + icon in horizontal deployment form
6.select applicable product name and then save
7.close that popup and again open same record
8.click on + icon under new change request number section
9.observe attachment fields
