Steps To Reproduce:

1. Log in to the FST.
2. Select the Technical safety concepts.
3. Click on the FTA and a Failure flow down.
4. Click on FMEDA.
5. Select the FTA and Add another failure for the same element.

Description

1. Add two failure Flow down under one parent element and add Failure flow down for one of the newly created failures in FTA, showing duplication in the FMEDA.
2. Added one Failure Flow down for an element, after refresh added new flow down for the same element, only the last added Failure flow down showing in the FMEDA.
