Steps To Reproduce:

1.Login EwQIMS
2.Goto New document request
3.Select the level and click choose file
4.Attach the document, enter the details
5.And click Add
6.System is not listing the uploaded documents after clicking Add
7.instead its refreshing by default

Description

New document request page is not listing the uploaded documents, getting refreshed by default-Impact of the fix ECS-44457 After uploading a document through New Doc Request, if the level is changed and we upload another Document then the system is throwing error
