New document Request--> When we giving all the fields are mandatory, the alert for the Change Description and Reason for change are interchanged

Steps To Reproduce:

1. Login EwQIMS as Doc pro user
2. Click on Doc pro settings
3. Enable and save the all fields are mandatory
4. Click on New document Request
5. Leave the change description as blank and click add button
6. Alert shows as Please enter "Reason for Request"
7. Leave the Reason for request as blank and click add button
8. Alert shows as Please enter "Please enter changes required"
