Steps To Reproduce:

1.Login as user with Request right only
2.Upload without attachment in New document Request
3.Login as Admin and goto Admin Actions and assign Author for the request
4.Login as assigned author and goto actions
5.Select Documents Needing Revision and click on the document and view the details page
6.The Reason & Change description field is interchanged
