Published PSW Form is showing 404 error

Steps To Reproduce:
1. Login as PM ins APQP/PPAP
2. Select a published PSW form from Deliverable info page or open from TOC
Description
1.Published PSW Form is showing 404 error.
2. 2nd revision document is showing no view access for the supplier contact

It works fine when we change the current URL in the config. Please check what URL you are sending as this is failing only in APQP
