Search functionality with respect to request date, approval date in the revision history summary is not working

Steps To Reproduce:

1. Login as module admin
2. Click on document menu
3. select the document TOC
4. Right click on the document number
5. click the Revision history summary
6. search the data by request date, approval date and effective date is not working

Description

1. search functionality is not working for request date and approval date
2. Label issue in request date , Instead of request date field, Duplicated two times approval date
