Steps To Reproduce:
1.Login to EwQIMS
2.Navigate to problem solver module
3.Click on Actions→Initial Problem Solving/Concern initial evaluation
4.Click on Concern title
5.Click on Delete icon near to attached file
6.Click on submit
7.Observe the Result
