Steps To Reproduce:

1.Loggin as FST module administrator

2.Click on Main and select any PI

3.Select FTA menu and select Design Failure from the drpdown

4.Now right click on the Diagram and select Add Probability option

5.Observe the FTA note label and safety mechanism table

6.Now add Safety Mechanism and click OK button

7.Observe the result
