Steps To Reproduce:
1.Login to Ewqims
2.Go to Doc pro
3.Create a folder and give full access to all the 3 columns for group
4.Create a new document request for the folder
5.Click on Documents and select the document in TOC
6.Right click and select add record
7.Attach the document and required details
8.Click Continue
Issue: The record is not created and the created records are in in conersion status
