Steps To Reproduce:

1. Login to EwQIMS and navigate to APQP.
2. Open an existing Project and add a new task.
3. Attach Aqua document and set MDR
4. Publish the Aqua draft and review it.
5. Publish the Aqua draft to convert to document.
6. Check the Revision History Summary
=> Aqua draft to document conversion becomes Revision 2 instead of Revision 1
