Published CR document requested date format is not as per the user preference.Also Logo icon missing in the published form(Published CR link in Change request Status menu and from TOC CR link column)(Initiate change req from Doc Pro)

Steps To Reproduce:

1.CLick on change req status and click on CN icon, check the date format(User pref is set as ddmmyyy)but it shows the date format in mmddyyy

1.Click on documents menu

2.Select the document which is revised from Change mgmt,

3.Check the CR link

4.View the document, check the requested date
