1.Page has double header

2.Breadcrumb is wrong, it should be concern forms>>Add concern but its showing as New concern forms>> New

3.Save icon is not in green colour on hovering

4.Added elements: Settings icon is not in proper place. Added elements screen is not good. More white space in RHS. For added elements options are not looking good + and - symbol and move symbol is not as per UI and its not properly placed

5.Add options for the element: Done button is not as per UI

6.Warning alert for same name should be yellow, whereas its showing green
