PS: Categories: Shows alert message to enter category name, but there is no such field called category name(same for sub categories menu). Checkbox shows mandatory, but allowing to save w/o checking on it. Need space b/w checkbox and text

PS: Categories:
1.Shows alert message to enter category name, but there is no such field called category name.Occurs when clicking on add and clicking save w/o entering name(same for sub categories and Problem classifications menu)

2.Checkbox shows mandatory, but allowing to save w/o checking on it.

3.Need space b/w checkbox and text

4.Categories,Sub Categories and Problem classifications menu: Need prompt label
