Form Service - Revision number is not getting updated

Steps To Reproduce:

1.Login as module admin
2.Open all form service related forms like Risk Forms
,Context Form, Custom Data entry forms in respective pages
3.Right click on form
4.Click on "publish document" button
5.Enter the data in mandatory fields
6.Click on OK button
7.After getting success message right click on form again
8.Click on "Publish document" button again
9.Click on OK button
10.Go to Docpro and open the respective folder and see the revision no

Description

Form Service - The Revision number is not getting updated for following forms. The revision number is showing always 1.
1. All Risk Forms
2. Context Form
3. Custom Data entry forms.
