Steps To Reproduce:
1.Login as BOSS module admin
2.Expand BOSS-Setup Menu
3.Click on "template" link above "Risk" and "Custom Forms Setup" label
4.Click on Add button
5.Enter % in form name and description field
6.Click on save button
7.Enter the % as search input on form name and description field
8.Select the different conditions by clicking on respective list
