VOC - Add Function Branch option is not working. So data cannot be added after refresh

Steps To Reproduce:

1.Login as RFD Module administrator

2.Click on any menu and then select the PI

3.Select Voice of Customer and Click Add button and refresh the page

4.Now right click on description column and select Add function branch option

5.observe the result
